not everything needs slots

Just a form: connect cards, prayer requests, and interest lists without slots

Every church we talk to runs two kinds of asks. There are the ones with spots: greeters, the meal train, nursery shifts. And there are the ones that just need answers: the connect card for visitors, the prayer-request box, “who's interested in the men's retreat?” The first kind lived on SignUpChurch. The second lived in Google Forms or Jotform, and the names in it never met the names in your People page. Today that ends.

The short version: On the New sheet screen, choose 📝 Just a form. Add your questions, publish, and share one link. Every response lands on the form's Responses tab, in your sign-up emails, in the CSV export, and on your People page. On Pro, each person syncs to your ChMS. Available on every plan, including Free.

Same questions, no slots

A plain form is the sign-up sheet you already know with the slots taken out. You get the same questions editor — short answer, dropdown, yes/no, required or optional, help text, and questions that appear only when an earlier answer calls for it — plus the same control over which contact fields the form asks for. The public page shows your church name, the title, and your questions with one Send button. The same link, QR code, and website embed work too, and the form design you've set applies.

Answers land where your people already are

  • The Responses tab lists every response with the answers, searchable, with a table view that gives each question its own column.
  • Your sign-up emails report new responses the way they already report sign-ups: instantly or in a daily or weekly roundup. Because a response is its answers, the email includes them.
  • Your People page picks up everyone who responds, so a visitor who filled in the connect card is on the roster the next time you invite your regulars to serve.
  • Your ChMS (Pro) gets the person, matched by email or created, exactly like a volunteer. One honest limit: nothing is ever scheduled from a form. A connect card doesn't put anyone on a Services plan, and forms can't be linked to a team, group, tag, or event.

The part we sweated: second responses

On an RSVP, answering again with the same email updates your earlier answer, because you only ever have one. On a form that's wrong: two prayer requests a week apart are two requests, and a family filling in the connect card for each visitor is three cards. So every send on a form is a new response. To change one, the person removes it with the link in their copy and sends a new one, which keeps the response you see exactly what they sent.

Moving a form over from Jotform or Google Forms? Build it as a plain form and your questions, conditional logic, and the names behind the answers all live in one place. Signature and file-upload questions are next on the roadmap.

The full walkthrough is in the help center: Build a plain form (no slots).

Common questions

What's a plain form, exactly?
A sheet with no slots. Pick “Just a form” on the New sheet screen, add your questions, publish, and share the link. People see your questions and the contact fields with one Send button — nothing to pick.
Where do the responses go?
The form's Responses tab, with the answers; your new-sign-up emails (instant or roundup), which include the answers; the CSV export on Basic and Pro; and your People page. On Pro, each person syncs to your ChMS like any volunteer, and the form can post to Slack.
Can someone respond twice?
Yes. Every send is a new response, so two prayer requests a week apart are two requests. To change one, they remove it with the link in their copy and send a new one.
Which plans include it?
All of them, including Free. A published form counts toward your plan's active-sheet limit like any other sheet.
